Left ventricular lead placement for cardiac resynchronization therapy (CRT) is challenging. Magnetic navigation system is a useful tool for catheter manipulation for radiofrequency catheter ablation. We observed one case of left ventricular lead placement using magnetic navigation system. A 74-year-old male patient suffered from dyspnea and syncope due to heart failure and ventricular tachycardia. We tried to implant a CRT-defibrillator in the patient. However, the left ventricular lead placement could not be placed in a proper vein. We used a magnetic navigation system for the left ventricular lead placement which made the procedure easy and safe.
